1. Any Staff that will not turn 18 before June 29, 2011 will receive an "Intent to Hire Form" with their contract.

2. Staff that will not turn 16 before June 29, 2011 will need to take this form along with a copy of the Special Limited Permit to Work to their school guidance counselor and have it signed. Both forms must be turned in when the Staff check in on Saturday June 19. You will need to take with you take it, along with a copy of your Birth Cerificate, Baptismal Certificate, Pasport, School Records or Drivers License to the School Department in the city/town where you live, and they will issue you either a Special Limited Permit to Work, or a Certificate of Age form.

3. Staff that will not turn 18 prior to June 18, 2011 will only need to take the "Intent to Hire Form" to their Guidence Counselor along with a "Certificate of Age Form". I completed Certificate of age must be turned in when the Staff check in on Saturday June 19. You will need to take these forms with you , along with a copy of your Birth Cerificate, Baptismal Certificate, Pasport, School Records or Drivers License to the School Department in the city/town where you live, and they will issue you either a Special Limited Permit to Work, or a Certificate of Age form.
